The Health Innovation Network AHSN South London invite you to our Advance and Urgent Care Planning Accelerator Conference

The Health Innovation Network (HIN) is the Academic Health Science Network for South London. In July 2020, the HIN were successfully awarded funding to deliver a programme to increase use and quality of shared electronic advance/urgent care plans using Co-ordinate my Care (CMC). The programme concentrated on clinical engagement.

Through in-depth interviews with clinicians and STPs/ICS leads and an analysis of CMC data, the HIN identified a number of opportunity areas to make a significant impact and drive uptake and quality use of CMC. The HIN supported eight projects in total, with at least one project in each ICS/STP area.

Although disrupted by the COVID19 pandemic all projects have made good progress and this event will showcase each of the projects, with project teams giving their reflections and sharing learning from their work. There will also be an opportunity to ask questions to each of the project teams.

The event is open for registration for health or social care professionals with an interest in advanced care planning and you are welcome to join any or all of the three sessions of the event – please see agenda details below.

10:00 – 11:35 Session One: Meet our first three project teams from Acute Trusts:

  • Project 1: Barking, Havering and Redbridge University Hospitals NHS Trust - Alison Drew (CMC Facilitator, Palliative Care Team), Heather Wright (Macmillan Palliative Care Team Leader), Dr Lucy Pain (Palliative Care Consultant)

  • Project 4: South East London Clinical Commissioning Group - Dr Emily Gibbs (GP and EOL lead in Southwark) and Joanne Hare (System Development Project Manager, Bexley and Greenwich)
    Title: Using advance care plans as decision support tools when care home residents need urgent care
     
  • Project 5: Balham, Tooting and Furzedown Primary Care Network (SW London) - Dr Kate Kandasamy (GP and EoL lead in Wandsworth), Penny Robson (Paramedic for Balham, Tooting and Furzedown PCN)
    Title: Using a paramedic in primary care to increase numbers of people with advance care plans.
