2025 NAHP COP Reversibility in Rehabilitation
When older frail people experience a health crisis their functional ability is significantly altered and yet it is at this point that their ability to live independently is judged. Decisions made during acute episodes have a lasting legacy on quality of life for the individual and their families as well as the economic impact. Social care currently operates on a ‘needs based assessment’ model which results in care packages delivered to meet needs regardless of the underlying cause and potential for change. It requires a high level of skill and detailed assessment to identify potential reversibility of a presenting level of function and engage appropriate services to optimise rehabilitation potential.
This session is a BGS communities of practice meeting. This meeting will focus on rehabilitation of older adults, particularly of interest for Nurse and Allied Health professionals.
The 90 minute session aims to cover 3 key areas:
- Review of function and the frailty model
- What are the key indicators that identify potential for reversibility of dependence in an acute episode
- What does rehabilitation look like with restricted input from community services
